What You’ll Do

As a Clinical Care Coordinator, you’ll be the linchpin between patients, families, and care teams—ensuring seamless care delivery from initial intake to follow-up. Your ability to assess patient needs, develop tailored care plans, and monitor outcomes will directly impact lives. Key Responsibilities

  • Coordinate care across medical disciplines to deliver comprehensive, patient-focused treatment.
  • Conduct clinical assessments and design individualized care plans that reflect patient goals and clinical needs.
  • Serve as a trusted liaison between patients, families, and providers to support clear, compassionate communication.
  • Monitor patient progress and ensure documentation meets regulatory and quality standards.

Requirements

What We’re Looking For

  • Education: Bachelor’s degree in Nursing, Public Health, or a related field.
  • Experience: Minimum of 2 years as a Registered Nurse, preferably in home health or care coordination.
  • Licensure/Certifications: Active RN license (if applicable) or healthcare coordination certification.
  • Skills:
    • Strong clinical judgment with a compassionate approach.
    • Excellent communication skills across diverse populations.
    • Proficiency in EHR systems and Microsoft Office.
    • Exceptional organizational and multitasking abilities.
  • Teamwork: A collaborative spirit with a proactive mindset—ready to adapt and advocate in a multidisciplinary setting.
