We are seeking a Direct Service Worker to join our team! You will assist in the daily care of elderly or disabled individuals.

Responsibilities:

  • Assist clients with daily living activities
  • Communicate ongoing care results and updates to relevant parties
  • Collaborate with clients and families for best care opportunities
  • Maintain a healthy and comfortable living environment

Qualifications:

  • Previous experience in personal care or other related fields
  • Compassionate and caring demeanor
  • Ability to build rapport with clients
  • Excellent written and verbal communication skills

Company DescriptionWe are an independent living agency that serves adults in San Diego County with intellectual/developmental disabilities, cerebral palsy, autism, epilepsy, and other conditions related to intellectual disability.
The support specialist will help assist the client with their Individualized Service Plan (ISP) goals and provide 1-on-1 training inside the consumer’s home or out into the community. The services you will be assisting the client with, but are not limited to:
Financial Management
Self-Advocacy Skills
Personal Care
Community/Recreational Resources
Mobility
Parenting Assistance
Household/Community Safety
Menu Planning
Health Awareness
Job Requirements:‬ Proof of High School Diploma, GED or Equivalent‬
At least 2 years work experience with individuals that have developmental disabilities.
The ability to program and work through client issues. And behavioral challenges
Applicant must have reliable transportation, proof of insurance, and registration
Writes individual objectives in measurable format
Writes quarterly reports reflecting goals and objectives
Follow through in a timely manner with personnel matters
Assist persons supported in meeting goals and objectives
Train person supported the skills required to meet goals and objectives
Assist person supported to develop relationships and find naturals and assist in accessing community resources
Maintain daily progress notes to reflect goals and objective
Provide crisis intervention as needed to person’s supported
Provide functional skills training
Secure self sustained living arrangements
Communicates all individual related information to appropriate relief staff in a timely manner
Must be able to pass all pre-employment assessments, physical, drug test and finger print review. You need to be able to perform administrative duties as well and attend all meetings.
